In order to facilitate Ontario Student Transcript requests for former
Halton DSB students, the following changes relating to standardization
of fees and processes across the board were implemented as of Monday,
July 17, 2006.
A standard board wide “request for transcript” form has been developed
and is available via the board
website (printable documents) or the documents can be picked up from
the Guidance Office.
A new fee structure has been approved for use by all schools which applies
to all transcript requests for former students. The new transcript
request fee is $20.00 for the first two copies of a transcript
(the fee is $20.00 even if the student only requires one OST) and
$5.00 for each additional copy. If paying by cheque or money
order, please make them payable to Georgetown District High School.
Schools have been advised that we are NOT to process transcript
requests for former Halton DSB students until receipt of
the completed “request for transcript” form and payment is received in
advance. For applicants who wish to have their OST mailed, a copy of their
photo id showing their date of birth must accompany the transcript request;
otherwise, the applicant must provide proof at time of pick up.
Please allow at least one week for transcript requests to be processed.
Transcript requests during peak times (September, February and June) may
be delayed. Former students who attended GDHS
prior to 1999 should be aware that these records are not in the current
school computer system and may require more time to produce. Please
note that transcripts need to be requested from the last school you attended
– (ie. If you attended Self-Reliant/Gary Allan H.S. after leaving GDHS
your transcript must be requested from them).
